Station House Dental Care provides dental services under an NHS contract. The Freedom of Information Act 2000 makes dentists providing NHS services public authorities in respect of information relating to those services. We have adopted the Information Commissioner’s Office (ICO) model publication scheme in full. This page is our guide to the information we routinely make available, how to get it, and what it costs.
The scheme covers information about our NHS services only. Information not listed here can still be requested in writing — we will respond within 20 working days, providing the information unless a legal exemption applies.
Class 1 — Who we are and what we do
Practice name, address, contact details and opening hours: on this website and in our practice leaflet, free of charge. Our clinical team, their roles, qualifications and GDC registration numbers: on our Meet the Team page. We provide NHS and private dental care. Our NHS services are commissioned by NHS Lancashire and South Cumbria Integrated Care Board.
Class 2 — What we spend and how we spend it
Information on NHS funding received by the practice and the cost of operating our NHS contract: available on written request from the practice manager. There may be a reasonable charge for printing and postage, stated in advance.
Class 3 — What our priorities are and how we are doing
Our priority is to provide safe, high-quality NHS and private dental care to our community. The practice is regulated by the Care Quality Commission; inspection reports are published on the CQC website, free of charge.
Class 4 — How we make decisions
Records of decisions affecting the provision of our NHS services: available on written request from the practice manager, where held.
Class 5 — Our policies and procedures
Published on this website free of charge: our Privacy Policy (including data protection, records retention and confidentiality), Complaints Procedure, Safeguarding Statement, Cookie Policy and Accessibility Statement. Other practice policies — including health and safety, infection control (HTM 01-05), equality and diversity, and staff recruitment — are available on request from reception, in hard copy, free of charge or at cost of copying.
Class 6 — Lists and registers
Any publicly available registers we are required to hold: available on request from reception. All clinicians appear on the GDC register, and our data protection registration appears on the ICO register.
Class 7 — The services we offer
Details of the NHS and private dental services we provide, NHS patient charges and our private fees: on this website (see Dental Fees) and in our practice leaflet, free of charge.
Charges
Information on this website is free. Hard copies may carry a reasonable charge limited to printing, photocopying and postage, which we will tell you about before supplying the information.
How to request information
Write to the Practice Manager, Station House Dental Care, Fernlea Avenue, Barnoldswick BB18 5DW, or email teeth@stationhousebarnoldswick.com with “Freedom of Information request” in the subject line. Requests for your own dental records are handled as subject access requests under data protection law instead — see our Privacy Policy.
If you are unhappy with how we handle a request, you can complain to us, and then to the ICO at ico.org.uk/make-a-complaint or 0303 123 1113.
